This program is designed to serve people who have gone through a recent loss (i.e., death of a spouse, onset of a disability/illness, etc…) and are affected to the point of impairment by the grief. It is based upon the concept of Interdependence. “Interdependence” means that people who are more connected with others in their community are more resilient, mentally and physically healthier, and report higher levels in their quality of life. Therefore, the goal of the program is to help the person identify and connect with friends, family, and community resources that will allow them to return to a level of interdependence that is effective in helping them to adjust to their new reality. And, to do so in a way that engenders long-term, non-paid relationships.
As part of our efforts to reach these goals with each person we support, we will be developing relationships with key community service agencies within the neighborhoods where the people we are supporting live. We will identify churches, community service providers, community centers, city recreation departments, etc… who enhance a sense of community in those neighborhoods and add value to the life of the people who live there.
Stages of Service Include:
- Assessment/Plan Development
- Fading Assistance
- Graduation from Services
